Inspector Notes

The Licensing Inspector initiated and completed an unannounced monitoring inspection on 6/20/2023. The inspector was on site from 2:30pm until 5:30pm.

There were 63 children under the care of 15 staff. The ages of the children ranged from five to twelve years old. The children were observed during indoor free play, movie time, snack time. Six children?s record, five medical record and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain repeat background checks after 5 years.

Evidence:
Staff # 4, with a hire date of 6/23/18, did not have an updated Sworn statement. The last sworn statement was dated 6/23/2018.
Plan of Correction: Sworn statement will be completed when employee comes in in the morning.
Standard 8VAC20-780-560-G
Based on observation, the center failed to ensure when food is brought from home the food container shall be sealed and clearly date and labeled in a way that identifies the owner.

Evidence: In main room with all children?s cubbies, there were children?s lunchboxes for the day. The lunchboxes had no names or date labeled on them.
Plan of Correction: All staff will be remined to label lunches when children come in in the morning.
