During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 06/05/2025. The on-site inspection began at 1:00 p.m. and ended at 1:45 p.m. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was six. The inspector reviewed two children?s records and one caregiver record on-site.

This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/13/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-A
There shall be documentation of a physical examination completed by a physician for each child prior to the child's attendance or within 30 days after the first day of attendance.

Child A and child B, who had been attending for eight months, did not have a physical on file.
Plan of Correction: New physical documentation for both children have been requested for parent to bring in and kept in record.
Standard 8VAC20-800-530-G
Pillows or filled comforters shall not be used for children under two years of age.

Two cots set out with blankets and pillows were observed in the day care area. It was reported that child A (age 13 months) would be sleeping on one of the cots with a pillow.
Plan of Correction: Pillow will be removed and no longer will be used for children under 2.
