Inspector Notes

An unannounced monitoring inspection was conducted on 1/9/2023. There were no children in care during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 2 staff records were reviewed. The inspector arrived for the inspection at 10:20am and departed the center at 11:53am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of two staff?s records, the center failed to ensure that an employee was not employed until the center had the person?s out of state sex offender registry completed and out of state criminal history name check for non-NFF only prior to the first day of employment.

Evidence: Staff #1 was hired on 8/23/2022 and on the day of the inspection did not have the required out of state sex offender registry results and out of state criminal history results for non-NFF states prior to the first day of employment.
Plan of Correction: Provider will obtain out of state sex offender registry and out of state criminal history results.

Based on observation, the center failed to ensure that annual approval from the health department shall be provided.

Evidence: The most recent annual health inspection was dated 10/19/2021 and on the day of the inspection did not have current annual approval from the health department.
Plan of Correction: Provider will obtain annual health inspection.
