Inspector Notes

An unannounced, on-site renewal inspection was initiated on 7/3/2025 and completed on 7/3/2025, as a part of the licensure period. The on-site inspection began at 10:30AM and ended at 1:18PM. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 5 children?s records on site on 7/3/2025 and 2 staff records on 7/3/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/21/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was due 7 months ago and Staff #2 updated screening was due 13 months ago. The updated screenings have not yet been completed.
Plan of Correction: Staff are scheduling TB screenings.
Standard 8VAC20-780-440-B
Cots shall be identified for use by a specific child. 10 cots were not labeled for use by specific children in classroom 1.
Plan of Correction: Cots are now labeled by staff.
Standard 8VAC20-780-450-A
Cots used by specific children other than infants shall have linens consisting of a top cover and a bottom cover or a one piece covering which is open on three edges. 6 children did not have a bottom cover and 1 child did not have a top cover and bottom cover in the classroom 1.

Pillows when used shall be covered with pillowcases. 1 child did not have a pillowcase in the classroom 1.
Plan of Correction: Parents are required to supply pillow cases.

When food is brought from home the food container must be clearly labeled and dated. 5 children?s lunches were not dated and 5 children?s lunches were not labeled or dated in classroom 1.
