Inspector Notes
A Subsidy Health and Safety Inspection (SHSI) was conducted on 12/7/2023 from 12:30pm to 2:00pm. A Voluntarily Registered (VR) renewal inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on observation and interview it was determined the vendor did not ensure that areas and equipment of the family day home, will be maintained in a safe, and operable condition.

A piece of linoleum that was approximately 4" in length, was peeled up and lifting at the entrance to the bathroom, posing a tripping hazard.
