Inspector Notes
A complaint inspection was conducted on February 13, 2023 in response to a complaint received by the Fairfax Licensing Office regarding behavioral guidance and staff training.
The inspection included classroom observations, interviews with staff, an inspection of the physical plant, and documentation review.
A preponderance of evidence for the complaint addressing the aspects staff training was found, and therefore the complaint is deemed to be valid. All violations related to both the complaint and other non-compliance can be found on the the violation notice.

Staff 1's record did not contain a sworn statement or affirmation.
Evidence:
Child 2's record did not contain documentation of immunizations.
Evidence:
The orientation documentation for Staff 1 does not have a staff signature of completion.
Evidence:
On the date of inspection there was only one staff member present in the role of supervising children. There was not an additional staff member present to manage the administration of the center.
Evidence:
1. The only staff member present on the date of inspection left the classroom where children were eating lunch to attend to administrative tasks of the center from approximately 11:19 am - 11:21 am.
2. During the inspection interviews and photos were reviewed which document one of the children present in the unattended room has a behavioral history of jumping off of tables.
Evidence:
Children's lunches were not labeled with the children's name or the date.
Evidence:
1. Child 1's record did not contain: the name, address, and phone number of one of the child's parents; the place and contact information for either parent's place of employment; documentation of the child's age and identity.
2. Child 2's record did not contain: child's date of enrollment; documentation of the child's age and identity.
3. Child 3's record did not contain the place and contact information for the listed mother's place of employment.
4. Child 4's record did not contain the address for one of the listed emergency contacts.

Staff 1's record did not contain: all required background checks; the results of two or more reference checks; the employee's date of hire; date of separation from employment.
