Based on record review, staff did not complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

The orientation documentation for Staff 1 does not have a staff signature of completion.

Based on observation, On the date of inspection during the center's hours of operation, there was not one adult on the premises in charge of the administration of the center.
Evidence:
On the date of inspection there was only one staff member present in the role of supervising children. There was not an additional staff member present to manage the administration of the center.

Based on observation, children under the age of 10 years of age were not within actual sight and sound supervision of staff.
Evidence:
1. The only staff member present on the date of inspection left the classroom where children were eating lunch to attend to administrative tasks of the center from approximately 11:19 am - 11:21 am.
2. During the inspection interviews and photos were reviewed which document one of the children present in the unattended room has a behavioral history of jumping off of tables.

Based on observation, food brought from home was not labeled with all required information.
