Inspector Notes

An unannounced monitoring inspection was conducted today to ensure compliance with the Intensive Plan of Correction (IPOC) that was entered into with the Department of Education on 10/11/2023.

The facility is not in compliance with the IPOC.

Another violation was cited that was not part of IPOC; however, found to be out of compliance during the inspection.

Based on observation and staff statements there was not always present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.
Evidence:
1. At the beginning of the inspection Staff 2 was the only staff member on the premises. One child was in care at this time.
2. Staff 2 called Staff 1 who returned to the center.
