Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on April 2, 2025. The inspection started at 10:00am and concluded at 11:00am. There were two children present and one caregiver; the point total was 3. The inspector reviewed three children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Children's information shall include permissions for field trips.

Child #1, Child #2, and Child #3's record did not contain documentation of permissions for fieldtrips.
Plan of Correction: The vendor will use the department's model form for permissions for fieldtrips and have the parents of each child to sign the permission form.
Standard 8VAC20-790-230-A
Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized.

Child #2 does not have documentation that they have been immunized in their record. Child #2 has been in care for 10 months.
Plan of Correction: The vendor contacted the parent of Child #2 at the time of the inspection to obtain a copy of the current immunizations. Moving forward, immunizations will be obtained before the child can receive care.

Caregivers who work directly with children shall annually attend at least 16 hours of training.

Caregiver #1 had documentation of nine of 16 hours of annual training.
Plan of Correction: The vendor will complete 7 hours of annual training.
