Inspector Notes
This unannounced Non-mandated Monitoring Inspection (NMI) was conducted on 02/22/2022 as a result of an incident reported by the center to the Office of Child Care Health and Safety (Fairfax).
Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the center.

Violations

Based on interview, the center did not ensure that the center's activities, services were maintained in compliance with the center's own policies and procedures (that are required by these standards, and the terms of the current license issued by the department).
Evidence -
1. The Center Director made a self-report to the Office of Child Care Health and Safety (Fairfax) of an incident that occurred on 02/22/2022 at approximately 10:15 am.
2. The Center Director stated (during an interview conducted on the date of inspection - 02/22/2022) that Child #1 (a four year-old) was left unsupervised in a classroom inside the center for approximately 1 minute.
3. It was also reported, that Staff #1 failed to follow center procedure by neglecting to administer a name-to-face roll call while transitioning out of the classroom. Center staff were instructed on name-to-face procedures during a training conducted (the day before) on 02/21/2022.
4. Once Staff #1 realized Child #1 was missing from the group, Child #1 was found in classroom hiding and unharmed. Child #1 was then returned to the rest of the group.
