Inspector Notes
The following inspection took place between the hours of 11 am through 11:30 am. Today, I met with the staff of the Purcellville Children's Academy. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an incident self-reported by the center to the Office of Child Care Health and Safety (Fairfax).
Information gathered during the inspection determined that there was (1) one non-compliance with applicable standard. Violations were documented on the Violation Notice issued to the center.

Violations
1Evidence -
1. The Director stated (during in interview conducted on the date of inspection - 05/19/2022) that on 05/18/2022, starting at approximately 5:21 pm, both Child #1 (4 years of age) and Child #2 (4 years of age) were left unsupervised within a center classroom (Frogs Room).
2. Staff #1 was supervising 4 preschool children on the playground. Two of these children needed to use the restroom. Staff #1 knocked on the fire-door of the Frogs room for the children to come inside to use their restroom. Staff #2 opened the door for Staff #1. Staff #2 then left to go to another classroom. Staff #1 attempted to provide supervision for the 2 children still outside and the 2 children now inside by standing in the threshold of the door. Staff #1 then allowed the door to shut with Staff #1 on the outside.
3. Child #1 was observed by Staff #3 alone in the Frogs Room (Child #2 was using the restroom). Staff #3 took Child #1 up to the front desk to Staff #4. Both Staff #3 and Staff #4 returned to the Frogs Room and found Child #2.
4. Based on video tape, the total amount of time that Child #1 was out of actual sight and sound supervision was 27 seconds. The total amount of time that Child #2 was out of actual sight and sound supervision was 45 seconds.
5. This incident was self-reported to the Office of Child Care Health and Safety (Fairfax).
