Inspector Notes

The following unannounced complaint inspection took place on 04/18/2024, between the hours of 2 pm through 3 pm, on site at the child day center located in the town of Purcellville, Virginia. Based on all available evidence, the following are the findings for those areas of the standard that were in review:

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the center.

Standard 8VAC20-780-270-A
Based on interview and review, it was determined that a piece of equipment was maintained in an unsafe condition.

Evidence -
1. On 04/08/2024, at approximately 10:30 am, Child #1 slid down the back of the changing table (from a standing position) on to the floor. Due to an opening in the back of the table, Child #1 sustained scratched to their back.
2. Normally, this table would have its back to a wall, however, the table was moved in order to provide better supervision of all children while staff changed diapers.
3. First Aid was administered to Child #1.
Plan of Correction: The changing table was removed from the room and a new table was ordered, obtained, and placed in the room the next day, 04/09/2024.
