Inspector Notes

An onsite announced initial subsidy health and safety inspection (SHSI) was initiated on January 19, 2022 and concluded on January 19, 2022. The licensing inspector was onsite from approximately 1:00pm until 2:30pm. There were five children present with two staff supervising. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-560-A
Based on the review of three children's records, the vendor did not receive immunization record required by the State Board of Health before one child began attending the center.

Evidence:
The record for Child #2 (first day of attendance 07/22/2021) contained immunization record dated 01/10/2022. The parent handbook stated prior to children attending the center, copies of immunization records must be obtained. A member of management confirmed this information during the inspection.
Plan of Correction: Now going forward, any newly enrolled child will be required to submit a complete immunization record prior to attending the center.
Standard 22VAC40-665-580-K
Based on review of two staff records and interview, the vendor did not ensure at least one staff was on duty who has obtained instruction in performing daily health observation of children within the last three years.

Evidence:
A member of management was asked if staff had obtained training in performing daily health observation of children within the last three years. The member of management stated no staff had been trained for daily health observation.
Plan of Correction: I was not aware of this requirement. All staff will complete the online training to perform daily simple health screening by the end of the week.
