Inspector Notes

The licensing inspectors conducted an unannounced Subsidy Health and Safety inspection on October 17, 2023 from approximately 9:45 AM to 11:30 AM. There were 31 children in attendance and a total of five staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the vender did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence:
Several areas of exposed landscaping fabric had worked its way to the playground surface. The exposed fabric created a tripping hazard.
Plan of Correction: We will remove the exposed fabric and add additional mulch to the areas.
