Inspector Notes
An announced modification Subsidy Health and Safety Inspection was conducted on Wednesday, March 13, 2024, from 2:00pm to approximately 4:39pm. The RECDC relocated to 113 Wilkinson Road, Richmond, VA. During the modification inspection, the facility was observed to be well laid out and filled with equipment for children. The classrooms have age-appropriate materials as well as tables, chairs, and other items for the children to use. The licensing inspector reviewed the occupancy, fire inspection, health inspection, and required posted documentation. The exemption permits services to 50 children in the age range from 2 years through 12 years, 11 months.
During the inspection, there were 38 children in attendance and a total of six staff directly supervising. Upon arrival, the children were in their designated classrooms resting. The children and staff were later observed walking to the cafeteria for an afternoon snack. They were later observed in their designated classrooms interacting with peers and staff. The children were engaged in various activities throughout the inspection. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, kitchen, hallways, outdoor play area, and bathrooms were inspected. Medication is administered and medications and authorizations were reviewed. Five children's records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence: The TB screening in the record of Staff #4, employed on 02/12/24, was completed on 12/17/23. The TB screening in the record of Staff #5, employed on 01/03/23, was completed on 01/18/22.
During interview, a member of management confirmed there were no other TB screenings on file for Staff #4 or Staff #5.
Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence: The most recent TB screening in the record of Staff #3, employed on 11/20/21, was completed on 11/11/21. A new TB screening should have been obtained no later than 11/11/23.
During interview, a member of management confirm that a new TB screening was not on file for Staff #3.
Evidence: During a walkthrough of the facility, the inspector observed unlocked cleaning materials in the hallway and in three classrooms.
