Inspector Notes

An initial inspection was completed on December 1, 2021. Center was reviewed to determine compliance to open a child day care center. Licensing inspector review two (2) staff records. The inspector reviewed compliance in the areas of administration, physical plant, programming, special care and emergencies. .

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Licensee failed to complete required Background Checks for any agent at the time of application who is or will be involved in the day-to-day operations or who is or will be alone with, in control of, or supervising one or more of the children.

EVIDENCE:
Licensing Inspector reviewed staff records. Record for Staff #2 did not have documentation of Fingerprint background check
Plan of Correction: Center Administrator will submit request to complete required fingerprint background check to the Licensing Inspector within 10 days
Standard 8VAC20-780-190-A-2
Based on record review, the Licensee failed to have documentation of the Center Director's qualifications.

EVIDENCE:
Licensing Inspector reviewed staff records. Documentation of qualifications for the Center Director were not received.
Plan of Correction: The center Administrator will submit transcripts & other required documentation to the Licensing Inspector.

Based on inspection, the Licensee failed to include the shelter-in-place procedures in the center's emergency preparedness plans.

EVIDENCE:
Licensing Inspector reviewed the center's emergency preparedness plan which did not include procedures for shelter-in-place emergencies.
Plan of Correction: Center administrator will establish shelter-in-place emergency plans & will submit them to the Licensing Inspector for review
