Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/18/2026. The on-site inspection began at 11:20 a.m. and ended at 1:35 p.m. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff 2 updated screening was due 2 months ago.
Plan of Correction: Staff will get a rescreen.
Standard 8VAC20-780-560-F
When centers chose to provide meals or snacks, the centers shall follow the most recent requirements of Child and Adult Care Food Program.

For lunch, the center served four food components. Five food components are required for lunch.
Plan of Correction: Provider will serve required components at every meal/snack.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child 4 (enrolled two months) did not contain documentation of viewing proof of the child?s identity and age.
Plan of Correction: Provider will obtain information from parent.
