This inspection was an unannounced follow up focused inspection to determine the progress of compliance of the previous violations found during the renewal inspection on 5/19/22.
Some areas have been addressed and are in compliance. Other areas are in the process of being compliant. The licensee will need to be fully compliant by the original submitted date of 6/13/22 in order to move forward with the renewal of the conditional license. Thank you for your time and cooperation.
