During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/23/2025 and completed on 4/28/2025. The on-site inspection began at 9:30 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 20 children present and 4 staff. The inspector reviewed 6 children?s records on-site and 4 staff records electronically on 4/28/2025. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-B
The physical examination shall be within the time period prescribed: within 12 months prior to attendance for children two years of age through five years of age.

The physical examination for Child 3, 4 years old, was completed 18 months prior to attendance.
Plan of Correction: Will ensure physical was completed within timeline.
Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who meets the requirements of medication administration training.

On 04/23/2025 for over 3 hours, Child 1, who has been prescribed emergency medication, was not in the care of a staff member that meets the requirements of medication administration training.
Plan of Correction: Provider will work on getting more staff MAT trained.
