Standard 22.1-289.031-A
The religiously exempt program is required to disclose to parents in writing the fact that it is religiously exempt from licensure and post the fact that it is exempt from licensure in a visible location on the premises.

Evidence: The record for Child #1, Child #2, Child #3, Child #4 and Child #5 did not contain documentation of a written disclosure signed by the parents that the program is religiously exempt and there was no posting that the facility is exempt from licensure.
Plan of Correction: Will include written notice to parents of the religious exemption status and will post the religious exempt certificate in a conspicuous place.
Standard 22.1-289.035-A
The religiously exempt program is required to obtain a sworn disclosure statement, central registry background check and criminal record check prior to employment and every five years thereafter.

Evidence: Staff #1?s sworn disclosure, central registry background check and criminal background check had not been repeated every five years, they were two years overdue.
Plan of Correction: Sworn Disclosure was signed on 10/30/25. The Criminal Record check and central registry search results were received on 11/5/25. Moving forward all background checks will be rechecked every five years before the expiration date.
