Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and an onsite tour of the program.

An initial inspection was initiated on 12/10/2021 and concluded on 12/10/2021. The director was contacted by email to initiate the inspection. There were no children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation it was determined that the center did not ensure that all areas of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The kitchen set in the classroom in the afterschool room was not anchored and presented a potential crush/fall hazard.
2. There were loose staples on the floor in the game room which presented a potential choking hazard.
Plan of Correction: Director will ensure that all floors are cleaned and the kitchen set is anchored.
