Inspector Notes

An unannounced monitoring visit and facility tour was conducted from 9:25 am- 10:11 am. At the time of inspectors' arrival there were 13 infant to preschool aged children in care with 3 staff members and 2 administrators. Children were observed interacting with staff, during free play, music and movement, and diapering/hand washing. A sample of 3 children's records and 1 staff record were reviewed. There have been no injuries. Director reported that there are no medications being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were be maintained in a safe.
Evidence:
1. There were 5 medium sized googly eyes on the floor in the 2 year old classroom presenting potential choking hazards.
2. Staff confirmed that there were googly eyes on the floor during the inspection.
Plan of Correction: Staff cleaned up the googly eyes off the floor.
Standard 8VAC20-780-290-A-3
Based on observation it was determined that the center did not ensure that all electrical outlets have protective covers in areas with preschool children.
Evidence:
1. There were two uncapped electrical outlets in rooms utilized by preschool aged children.
2. Staff confirmed these outlets were uncapped during this inspection.
Plan of Correction: Staff capped the outlets during the inspection.
Standard 8VAC20-780-500-B
Based on observation and interview it was determined the center did not ensure when a child's diaper becomes wet or soiled, the child is cleaned and changed immediately.
Evidence:
1. There was a strong odor of a soiled diaper in the two year old room. This inspector mentioned it to the lead teacher in the room, who was unaware of the odor.
2. The lead teacher did not check the diapers of the children in the room. The Assistant Director came into the room a few minutes later, was informed about the concern by this inspector, then checked the children's diapers and changed the child with the soiled diaper.
Plan of Correction: Staff will be reminded to check diapers when there are odors as well as on the regular scheduled intervals. Staff will be reminded to call for a child to be changed immediately upon discovering their diaper is soiled.
