Inspector Notes

A Subsidy Health and Safety inspection was conducted on 09/11/2024 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/23/24. A POC submitted after this date will not appear on the public website.

Based on record review and interview, the vendor did not ensure that all had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. The record for staff #1 (hired 5/13/24) did not contain documentation of CPR certification.
2. Staff confirmed that staff #1 did not have CPR certification.
