Inspector Notes
An unannounced, on-site monitoring inspection was completed on 4/3/25, as a part of the licensure period. The on-site inspection began at 1:05 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
If your plan of correction (POC) had not been provided during the exit interview of the inspection, please complete POC and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/15/25. A POC submitted after this date will not appear on the public website.

10The most recent criminal history check for staff #2 was overdue by a year and seven months.
There was no immunization updated for child #1, it was overdue by 2 years and 4 months.
Staff #5's updated screening was due 1 year and 4 months ago.
Staff #2, #3, and #5 did not have the 16 required hours in 2024.
In the hallway to the two year old classroom, there were five cleaning solution/sanitizing solution in an unlocked closet, out of reach of children.

During the inspection, two to five year old children were unsupervised on four separate occasions. 4 two year old children children were unsupervised for approximately three minutes each on two occasions and 8 3-5 year old children were unsupervised for approximately five minutes.
Staff #1 was alone supervising the five children in the infant room at the time of this inspectors arrival.
The record for Child #1, enrolled for three years, was missing the following required components: the city and state for the 2 required emergency contacts.
The record for Child #3, was missing the following required components: 1 of the two2 required emergency contacts, first date of attendance, and proof of age and identification.
The record for Child #4, was missing the following required components: the city for 1 of the 2 required emergency contacts, and first date of attendance.
At approximately 1:05 pm on 4/3/25, there were 17 children present in the center, but only 10 were marked present on the attendance record.
