Inspector Notes

An unannounced, on-site monitoring inspection was completed on 10/20/2025, as a part of the licensure period. The on-site inspection began at 12:00 pm and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

If your plan of correction (POC) had not been provided during the exit interview of the inspection, please complete POC and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/12/2025. A POC submitted after this date will not appear on the public website

The center is required to have employees undergo a background check every five years.

The most recent criminal history check for staff #2 was overdue by over two years.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Child #5, almost three year old, was in a crib through out this inspection. Child #5 previously broke the bottom out of a crib identical to the one they were in during this inspection.

The crib occupied by child #4, a seven month old, had a broken mattress board. The mattress was folding in around the perimeter due to the mattress board being broken. Staff stated that the mattress board had been previously broken by child #5.

These cribs were unsafe for child #4 and #5 to be in.

Standard 8VAC20-780-40-L
The center shall inform all staff who work with children of children's allergies, sensitivities, and dietary restrictions.

Child #1, in care during inspection and enrolled for two weeks, has a diagnosed allergy to peanuts and milk. Staff stated that they did not have any children enrolled with food allergies and were unaware of child #1's allergies.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

There was no allergy list on site, child #1 has peanut and milk allergies.

Furnishings, equipment, and materials shall be of an appropriate size for the child using it.

The crib being utilized by child #5, an almost three year old child, was not appropriate to be used by this child.

Standard 8VAC20-780-450-A
Cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

In the two to three year old room there were seven children on cots, five did not have bottom covers, and all seven did not have top covers.

The record for each child enrolled shall contain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
