Inspector Notes

An unannounced monitoring inspection was conducted on 11/16/2023 from 1:20pm to 2:30pm with the provider. There were three children in care, ranging in age from 14-months to four-years-old, for a total of eight points. Children were observed napping and having free play. Three child records, the provider record, and five household member records were reviewed. The provider has current certification in CPR and First Aid. The provider does not give medication and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication in the future. The emergency drill log, attendance, and menu were also reviewed. Based on review of caregiver and household member records, the provider did not obtain a current tuberculosis (TB) screening from each caregiver and adult household member every two years from the date of the first screening. Evidence: The most recent TB screenings on record for Caregiver A and Household Member D were dated 11/12/21. The most recent TB screening on record for Household Member B was dated 9/16/21. The most recent TB screening on record for Household Member E was dated 9/17/21.

Based on review of three child records, the provider did not obtain documentation that a child had been adequately immunized before the child attended the family day home. Evidence: The record for Child A, with a start date of 10/5/23, did not contain documentation of immunizations.
