Inspector Notes
Thank you for your assistance during this announced initial inspection. This on-site visit was conducted on January 13, 2022 from 11:20 a.m. through 12:15 p.m. At the time of the inspection, four children were in the care ranging in ages from 19 months through 2. Points totaled 13. The records were reviewed for the provider and 4 children.
Areas reviewed today were as follows: Indoor and outdoor areas of the home, emergency supplies, posted information, equipment used for the children in care, and supplies. There are a wide variety of materials for the children in care.
A recommendation will be made for issuance of a six (6) month conditional family day home license.

1Evidence:
1. Emergency supplies were reviewed.
2. Two triangular bandages were not found in the emergency supply kit.
3. The provider confirmed she did not have two triangular bandages.
