The provider did not practice emergency evacuation drills monthly. An emergency evacuation drill was not practiced in December 2025.
The provider did not make sure the number of children receiving care did not exceed more than four at any one time. There were five children present during the inspection.
The record of Child #5, who has been enrolled for over 10 months, did not include documentation that proof of identity was reviewed and the date of review. The required signed acknowledgement of written notification required by ? 22.1-289.050 of the Code of Virginia was not on file during the child's attendance on 01/05/26.
