Inspector Notes

A monitoring inspection was initiated on March 29, 2022 and concluded on April 4, 2022. There were thirty-eight children present, ranging in ages from infants to four years, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A-1
Based on review, facility did not ensure 1 of 3 staff have a completed TB test or screening prior to coming into contact with children.

Evidence: The record for staff #3 (start date March 21, 2022) did not have a TB test or screening.
Plan of Correction: Staff will have test or screening completed. In the future, TB will be completed prior to beginning employment.
Standard 8VAC20-780-430-K
Based on observation, facility did not ensure there was a provision for an individual place for each child's personal belongings.

Evidence: Three backpacks were observed on the floor in the 2's classroom.
Plan of Correction: Additional cubbies were provided so that each child has a place for their personal items.
Standard 8VAC20-780-450-D
Based on observation and interview, facility did not ensure when pillows are used that they are assigned for individual use and covered with pillow cases.

Evidence: On March 29, 2022, 7 of 12 children in the preschool room were observed napping with pillows that were not assigned for individual use and not covered in pillow cases. Staff reported 6 of the pillows were supposed to be used on the classroom couches.
Plan of Correction: Staff were reminded that children cannot nap with the classroom pillows.
Standard 8VAC20-780-500-B
Based on observation, facility did not ensure diapers are disposed of in a diapering system that is not hand operated or requiring the diaper to touch the exterior surface of the system.

Evidence: The diaper pail in the infants 2 room did not operate with the step therefore someone would have to manually lift the lid with their hand in order to dispose of a diaper.
Plan of Correction: The lid was reattached properly. Staff were reminded to keep the lid securely attached in all diaper pails.
