Inspector Notes
A complaint inspection was initiated on October 26, 2022 and concluded on December 8, 2022 regarding programs. The inspector reviewed compliance in physical plant, staffing and supervision, programming and special care/emergencies. Documentation related to one child was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Evidence: An injury report completed on 10/11/2022 did not document a second signature whether it be a staff or parent.
