Inspector Notes
An unannounced monitoring visit was conducted for the after school program. Thirteen school-age children were present with two staff which is within ratios for supervision. The program is located in the trailer at the local elementary school. Observed the end of circle time and snack, transition to the restroom inside the school building, assessment for tutoring and read aloud time by a staff member. Reviewed staff trainings, emergency preparation, medication procedures (none on-site), physical space and activities. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
