During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 12/03/2024, as a part of the licensure period The on-site inspection began at 3:15pm and ended at 4:15pm. The inspector reviewed compliance in the areas listed above. There were 15 children present and 4 staff. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/17/2024. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

No documentation that the center has obtained or requested a search of the child abuse and neglect registry from a state where Staff #3 has lived in the past five years. Staff #3, who has been employed for more than nine months and was working with children at the time of the inspection.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

An open container of cleaning wipes that are considered hazardous were in the classroom and accessible to children.

Narcan medication found in an unlocked drawer, accessible to children.

A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #1, who has been enrolled for six weeks, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: The center is in communications with parents about a plan.
