Inspector Notes

An unannounced monitoring inspection was conducted during the Conditional licensure period. Sixteen children were present with two staff which is within ratios for supervision. Observed reading, group time and book club. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain fingerprint results prior to employment.
Evidence:
Staff #1 documented date of hire was 8/15/2022 and the fingerprint results were dated 8/17/2022.

Based on record review, a staff member did not obtain a TB screening within the required time frame.

Based on staff statement and record review, the center allows children to leave the center unsupervised.
Evidence:
When observing the children's records on-line it was noted that several were listed as walkers. When the Licensing Inspector questioned the Director she stated that they left the center and walked home.

Evidence:
The center capacity is 16 children. However, the center has enrolled 18 students the week of 9/12-9/16 and on 8/30- 17 children were present.
