Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations. The documentation of immunizations obtained for Child #1 was dated more than three weeks after the child's first date of attendance.
Plan of Correction: Vendor will ensure documentation of immunizations are obtained before a child begins attending the family day home.
