During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on December 5, 2024. The inspection started at 11:30 a.m. and concluded at 12:30 p.m. There was one child present and one caregiver; the point total was two. The inspector reviewed two children?s records and five caregiver/household members records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The child?s information should include the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.

The record for Child #1 (in care for over seven months) did not have an address listed for their emergency contacts.
Plan of Correction: The information will be recorded.
Standard 8VAC20-790-430-A-3
Lockdown procedures shall be practiced at least annually.

The last record of a lockdown procedure was March 7, 2023 which was overdue by over eight months.
Plan of Correction: The lockdown drill was completed in September, but was not documented. I will be sure to document it.
