Inspector Notes

A monitoring inspection was conducted as a follow-up to the issuance of a conditional license. The inspection was initiated on April 26, 2022 and concluded on April 26, 2022. The director was present and participated in the inspection. There were 85 children present, ranging in ages from five months to ten years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, and nutrition. A total of five child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center did not ensure prepared infant formula is refrigerated, dated and labeled with the child's name.

Evidence:

Two bottles for child #6 were refrigerated and labeled with the child's name but were not dated as required.
Plan of Correction: Per the administrator: We will ensure all bottles are dated along with the name.
