Inspector Notes
An unannounced monitoring inspection was conducted on October 5, 2023 from approximately 11:15 am to 1:15 pm. There were 15 children in attendance and a total of 5 staff present. 3 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed taking a nap while the licensing inspector was on-site.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 10/19/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
12Evidence: The record for staff #3, indicated that they have lived in another state within the 5 years preceding the date of employment. The record did not contain documentation of a criminal history information check, a sex offender registry check, or a search of the child abuse and neglect from that state. Staff #3 was observed working in a classroom alone with children.
Based on record review, and observation, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and no more than 30 calendar days prior to employment.
Evidence:
1. The record for staff #2, date of employment 08/14/23, did not contain documentation of a negative tuberculosis screening. Staff #2 was observed working in a classroom with children.
2. The record for staff #3, date of employment 08/08/23, contained documentation of tuberculosis screening dated 03/12/23.
Based on record review, the center failed to ensure that staff complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence: The record for staff #2, date of employment 08/14/23, did not contain documentation of orientation training. Staff #2 was observed working alone in a classroom with children.
Evidence: The health inspection was dated 08/01/2022. Annual approval from the health department was due on or before 08/01/2023.
Based on record and documentation review, the center failed to maintain a current written list of children's allergies, sensitivities, and dietary restrictions.
Evidence: The record for child #5, date of enrollment 08/28/23, contained documentation of a food allergy. The allergy list did not have child #5 listed.
Evidence: Staff #2 was observed changing a child's diaper. Staff #2 did not wash their hands or the child's hands before or after the diaper change.
Evidence: Staff #2 was observed changing a child's diaper on the diaper table. Staff #2 wiped the surface with an antibacterial wipe after the diaper change. Staff #2 did not clean the surface with soap water and then sanitize with a spray or dip.
Evidence:
1. The record for staff #2, date of employment 08/14/23, contained documentation of CPR dated 09/16/21. The training certification period ended on 09/16/23. Staff #2 was alone in a classroom with children.
2. The record for staff #3, date of employment 08/08/23, did not contain documentation of CPR training. Staff #2 was alone in a classroom with children.
Evidence:
1. The record for staff #2, date of employment 08/14/23, contained documentation of first aid dated 09/16/21. The certification period ended on 09/16/23. Staff #2 was alone in a classroom with children.
2. The record for staff #3, date of employment 08/08/23, did not contain documentation of first aid training. Staff #2 was alone in a classroom with children.
Evidence: The last drill documented was dated 06/15/2023. When asked, staff #4 stated that drills are not implemented monthly.
Based on observation, the center failed to ensure that when food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner.
Evidence: The infant room contained bottles and containers of food in the refrigerator that were not labeled or dated.
Based on record review, the center failed ensure that staff records contain all required information.
Evidence:
1. The record for staff # 2, date of employment 08/14/23, did not contain two or more references as to character and reputation as well as competency prior to employment.
2. The record for staff #2, date of employment 08/14/23, did not contain information about any health problems that may interfere with fulfilling the job responsibilities.
3. The record for staff #2, date of employment 08/14/23, did not contain the name, address, and telephone number of a person to be notified in an emergency.
