The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was a play pen located in the playroom that was broken. One side of play pen would not stay up, creating potential entrapment or entanglement hazards.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. Children in the Toddler room were observed in a classroom that had an extension cord with two uncovered outlets that were within their reach.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. During the inspection, Staff #2 and Staff #4 were observed stepping in and out of the classroom multiple times, leaving 15 children (ages 18 months to four years) unsupervised.

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group. During the inspection, Staff #2 and Staff #4 were observed supervising 15 children. The youngest child in the group was 18 months old. The required ratio for children ages 16 to 24 month is one staff per five children.

REPEAT VIOLATION
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for 13 months, was missing documentation of child updates and confirmation of up-to-date information in the child's record. The record for Child #2, enrolled for one day, was missing the address of two designated people to call in an emergency if a parent cannot be reached. The record for Child #4, enrolled for three weeks, was missing the parent?s work phone number. The record for Child #5, enrolled for three weeks, was missing the address of one custodial parent.
