Inspector Notes

A monitoring inspection was conducted on December 5, 2022. There were 6 children present, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of 3 children's records, the provider did not obtain documentation of a physical examination prior to first day or within 30 days after their first day of attendance.
Evidence
The provider did not obtain documentation of a physical examination for child #2 within 30 days of their first day of attendance, 10/31/22.
Plan of Correction: I will get a copy of their physical examination and will make sure to get one within a child's first 30 days of attendance.
Standard 8VAC20-800-90-A
Based on review of 3 children's records, the provider did not obtain documentation that the child has been adequately immunized before the child's first day of attendance.
Evidence
The provider did not obtain an immunization for child #1 before their first day of attendance 7/5/22.
Plan of Correction: I will get a copy of their immunization record and make sure I get the immunization record before a child's first day of attendance.
