Inspector Notes

A monitoring inspection was initiated on 01/24/2023 and concluded on 01/24/2023. There were 18 children present, ranging in ages from 3 years to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed/updated.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of children files, the center failed to ensure that immunizations were received before or on the first day of attendance.

Evidence:
1. Date of enrollment for Child B was 01/04/2023. There are no immunization documents in Child B's file.
2. Staff verified that documents of immunizations are not in Child B's file.
Plan of Correction: I contacted the parent and will get the immunizations as soon as possible. I will make sure that I have those before the first day of enrollment.
