Inspector Notes

An unannounced renewal inspection was initiated and concluded onsite on October 17, 2022. There were 101 children in care ages two through five years old with sixteen staff and one administrator. Children were observed in teacher and self-directed activities including outside play, classroom play activities, transitions, physical education, restroom routines, hand washing lunch, nap and parent pick-up. The walking tour of the center inside and outside was completed. Children and staff records were reviewed. Annual inspections, emergency drills, emergency equipment, medication and transportation were reviewed. Violations regarding the timing of background checks appear on the violation notice and discussed with owner during an exit interview at the conclusion of the inspection.

Based on record review and interview the center failed to ensure that fingerprint background checks are obtained prior to employment.
Evidence:
1 - The record for Staff #2 contains a fingerprint background check dated September 28, 2022. However the documented date of hire for the staff was March 1, 2022.
2 - The record for Staff #3 contains a fingerprint background check dated September 9, 2022. However the documented date of hire for the staff was September 1, 2022.
3 - The record for Staff #4 contains a fingerprint background check dated June 17, 2022. However the documented date of hire for the staff was June 2, 2022.
4 - Staff 1 confirmed that the fingerprint background checks were obtained after the documented date of hire.
Plan of Correction: Going forward center management will ensure that fingerprints are obtained prior to employment. Hire dates will be documented as of the first time a staff clocks in for payment of services.

Based on record review and interview the center failed to ensure that and employee must not be employed until the center has the person's completed sworn statement or affirmation.

2 - The record for Staff #3 contains a sworn statement or affirmation dated September 3, 2022. However the documented date of hire for the staff was September 1, 2022.
3 - The record for Staff #4 contains a sworn statement or affirmation dated June 6, 2022. However the documented date of hire for the staff was June 2, 2022.
4 - Staff 1 confirmed that the sworn statement or affirmation for each staff listed were obtained after the documented date of hire.
