Inspector Notes

A renewal inspection was conducted on 11/23/2022 with center director and staff. There were 24 children present, ranging in ages from 3 months to 12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 1 staff record were reviewed. The children were eating lunch, watching TV, and playing with play dough.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on review of one new staff records, the center failed to obtain documentation of a negative tuberculosis (TB) screening or test for each staff at the time of employment and prior to coming in contact with children. Staff A (date of employment 9/12/2022) had a TB test on 9/28/2022.
Plan of Correction: Tuberculosis tests will be conducted prior to employment moving forward.
Standard 8VAC20-780-340-A
Based on staff interviews, the center did not ensure when staff are supervising children, they always ensure their care, protection, and guidance. On 11/23/2022, a 3 month old infant was asleep in their car seat. The infant's head was faced down and to the side, with a blanket close to the mouth and nose.
Plan of Correction: The infant was moved to the crib.
Standard 8VAC20-780-360-A
Based on observation, the facility failed to have a variety of age and state appropriate activities. Evidence: the infant room had a movie on the television during lunch and rest time. Television is not recommended for children under the age of two years.
Plan of Correction: Television will be used with discretion in all classrooms.
Standard 8VAC20-780-440-L
Based on observation, the facility failed to prevent the use of stuffed animals and filled comforters for children under the age of two years. Evidence: In the infant room, there were thick blankets in two cribs and a stuffed animal in one crib.
Plan of Correction: Blankets, stuffed animals, and soft bedding will not be used in cribs.

Based on review of one new staff record, the facility failed to have required documentation in each file. Two references as to character, reputation, and competence should be checked prior to employment. Evidence: Staff A (date of employment 9/12/2022) did not have documentation of two references on file.
Plan of Correction: Moving forward, two references will be documented for each staff prior to employment.
