During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was conducted on November 25, 2024 from 1:30 p.m. to 3:45 p.m. The inspector reviewed compliance in the areas listed above. There were 31 children present and 7 staff. The inspector reviewed 5 children?s records and 3 new staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 9, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-430-B
The facility shall provide equipment that is age and stage appropriate for the children. Evidence: Fourteen preschool children were sitting on the floor and standing at tables eating afternoon snack. The chairs were stacked on the side and not available for the children to sit and eat.

Standard 8VAC20-780-490-E
When any surface has been contaminated with body fluids, it shall be cleaned and sanitized. There was a bag of soiled clothing from a child's toileting accident on the children's bathroom sink. The clothing had not been removed and the area had not been sanitized.

Repeat violation. Staff shall wash hands with soap and running water before and after a diaper change. Staff A did not wash hands before or after changing a child?s diaper.
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff A changed a child?s diaper in the infant room and did not wash the child's hands with soap and water.

Bottles shall not be used while the child is in his designated sleeping location. There was a bottle in the crib with a sleeping infant.
