Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring initial inspection was initiated on January 11, 2022 and concluded on January 12, 2022. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, programming, medication, special care and emergencies and nutrition. A total of 1 staff record and the background checks for 5 individuals were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-40-N
Based on documentation review and staff interviews, the licensee did not ensure to develop a written playground safety procedures that includes all required items.

Evidence: The playground procedures available for review did not include the following information;
1) Provision for active supervision by staff to include scanning play activities, and circulating among children.
2) Method of maintaining resilient surface.
Plan of Correction: Program Director will update playground procedures.
Standard 8VAC20-780-540-C
Based on observation nd staff interviews, the licensee did not ensure to have all required items for the first aid kit.

Evidence: The first aid kit available for review during the inspection did not include a triangular bandages.
Plan of Correction: Program director will purchase triangular bandages for first aid kits.
Standard 8VAC20-780-550-A
Based on documentation review and staff interviews, the licensee did not ensure to develop a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and relocation, shelter-in-place, and lockdown. The plan, which shall be developed in consultation with local or state authorities, shall include the most likely to occur emergency scenario or scenarios, including fire, severe storms, loss of utilities, natural disaster, chemical spills, intruder, violence at or near the center, terrorism specific to the locality, and other situations, including facility damage that requires evacuation, lockdown, or shelter-in-place.

Evidence: The emergency plan procedures available for review did not include the following information;
1) Establishment of center emergency officer and backup officer to include 24-hour contact telephone number for each
2) Assembly points, relocation site and secondary means of egress, and complete evacuation of the buildings
3) Staff training requirement, drill frequency, and plan review and update
