Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on March 14, 2022 and concluded on March 16, 2022. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 36 children present, ranging in ages from 3 to 5, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Based on record review and staff interviews, the licensee did not ensure that at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence: Two out of three records reviewed did not have updated TB screenings. Staff #1's most recent TB screening was dated 2-18-2019 and Staff #2's most recent TB screening was dated 3-14-2020.
Plan of Correction: Human Resources has scheduled appointments with the agency provider for both mentioned employees. The appointments will be on 03/17/2022 and on 03/18/2022. The provider will send the records electronically to the agency once completed. Expected date is 03/21/2022. The updated records will be sent to licensing once they are received from the provider.
