Inspector Notes

A monitoring inspection was initiated on February 15, 2023 and concluded on February 16, 2023. There were 17 children present, ranging in ages from 3-5, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on review of 5 staff records and staff interviews, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening and the documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: Staff #5?s date of hire was 9-19-2022 and the TB screening/ test was dated 12-13-2021 and it was not conducted within the required timeframe.
Plan of Correction: The staff member has been sent to the agency's medical provider for a revised TB screening and will be completed by March 3, 2023. All future new hires will be sent to the agency's medical provider for a TB screening within the required thirty days.
