During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/14/25. The on-site inspection began at 12:45pm and ended at 2:15pm. The inspectors reviewed compliance in the areas listed above. There were 36 children present and 8 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

There were two cans of sanitizing spray in an unlocked cabinet in the hallway bathroom used by children. The cleaning materials were accessible to children in care.
Plan of Correction: The center responded with the following: A new lock was put on the cabinet during the inspection.

The required ratio for children ages 16 to 24 month is one staff per five children.

Staff 1 was observed alone in the classroom supervising seven children (youngest child 16 months old) during the transition period between lunch and nap time.
Plan of Correction: The center responded with the following: One of the staff in the tots classroom left for break while the classroom was in transition between lunch and nap time. Staff will be retrained regarding classroom ratios and will be reminded that the children should be on their cots before starting breaks.

The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Staff 2 cleaned the diaper changing surface a soap and water solution but did not sanitize the surface.
Plan of Correction: The center responded with the following: Staff will be retrained on proper cleaning procedures of diapering surfaces.

Documentation of lockdown drills shall be maintained by the center.

There was no documentation of a lockdown drill being practiced for the year 2024.
Plan of Correction: The center responded with the following: Management will be reminded that all practice drills need to be documented on the drill log.

Staff shall sit with children during meal times.

There were 3 childen in the Tots classroom eating lunch at a table and there was no staff sitting with them.
Plan of Correction: The center responded with the following: Staff will be reminded that they are to sit with children at meal times until all children are finished eating.
