Inspector Notes

A 60-day inspection was initiated on6/08/2022 and concluded on 6/08/2022. There were a total of 6 children with 2 staff members present. Children were observed during eating breakfast (quesadilla, apple, tangerines and milk) watching Mickey Mouse. The children transition to circle time and then outdoor play.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on children records review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2 did not have a physical examination.
Plan of Correction: I have scheduled a doctor app for Child #2.
Standard 8VAC20-800-60-A
Based on records review, the provider did not maintain an up-to-date record at the family day home for each enrolled child.
Evidence:
1. Child #1 did not have proof of child's age and identity.
Plan of Correction: I ask the mom for the birth certificate.
Standard 8VAC20-800-760-A-1
Based on emergency supplies review, the provider did not obtain a first aid a minimum of first aid supplies in the first aid kit.
Evidence:
1. Triangular bandages, First Aid Instructional Manual, thermometer, gloves were missing in the first aid kit.
Plan of Correction: I bought a new first aid kit and it has the triangular bandage, first aid instructional manual, etc.
