Inspector Notes
An unannounced monitoring inspection was conducted on 6/10/2024 at 11:10am. There were 7 children (22 points) directly supervised by 3 caregiver(s). The home, 3 caregiver records, 4 child's records, evacuation drills, emergency supplies, and policies were inspected. There were no children's medications on site today. Children were observed playing freely inside the play room, coloring, coloring, preparing for lunch, and eating lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence: observed a surge protector with 5 uncovered outlets in the main play room, accessible to children.
Evidence: reviewed 4 children's records and found the following:
1) Child 1 did not have a second emergency contact and Child 4 did not have an address for the second emergency contact.
2) Child 1 did not have documentation of work information for both parents.
3) Child 1, 2, and 3 did not have documentation of medical insurance numbers.
4) Child 2 did not have documentation of the parent's annual review and update of the emergency contact information. The was no date on the enrollment record.
5) Child 4 did not have documentation of signed emergency medical authorization.
Evidence: observed that the provider did not have a menu for the current one-week period.
Evidence: observed that the temperature in the refrigerator was at 41 degrees F and the temperature in the freezer was at 5 degrees F.
