Inspector Notes
An unannounced renewal inspection was conducted on 7/15/22. There were 4 child care children and 3 of the provider's own children (13 points) directly supervised by a caregiver. The home, 1 staff record, 4 children's records, 1 household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Violations
6Evidence: reviewed four children's records and found that Child A did not have documentation of a physical examination.
Evidence: observed that the provider's toaster was plugged in in the the main level kitchen, while she was providing care in the basement.
Evidence: observed that there was 1 uncovered electrical outlet in the basement play room.
Evidence: identified through the provider interview that she did not keep a written record of attendance this week, nor the previous week.
Evidence: Reviewed 4 children's records and found that child all 4 children did not have documentation of signed acknowledgement of the provider's medication policy.
Evidence: reviewed 4 children's records and found that Child C did not have documentation of immunizations.
