Inspector Notes

An unannounced renewal inspection was conducted on 08/21/2023. There were 5 children, ages 6 months to 5 years, totaling 13 points and one caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, household members, training, equipment and environment, preventing the spread of disease, emergencies, nutrition and background checks. A total of 5 children?s records, one caregiver/applicant record, and one household member record were reviewed. The children were observed during self-directed activities. The inspector arrived for the inspection at 9:00 am and departed at 10:30 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the provider failed to ensure that prior to a child?s attendance or within 30 days after the first day of attendance, documentation of a physical examination by or under the direction of a physician shall be obtained.

Evidence: The record for Child 2 contained a documented start date of 12/19/2022. The physical was dated 06/05/2023. The physical was not obtained within the 30-day timeframe.
Plan of Correction: Provider will obtain physicals within 30 days of enrollment.
